Drug
and Alcohol Evaluations/Assessments:
Often when an individual is charged with a DUI (or any other
drug or alcohol related charge), they will be required to
undergo an evaluation either by a
judge, attorney or probation officer.

Anger
Management Evaluations/Assessments:
A judge, attorney or probation officer will often require an
individual who has been charged with domestic violence, simple
battery or assault to also undergo an anger management
evaluation.

What To
Expect:
If you are coming in for a court ordered evaluation you will
fill out a few forms and a questionnaire. Then we will talk
about your situation (this will take about an hour).
Approximately one week or less after our meeting, provided I
have all the information I need, I will send a
typed report to you and your referral source such as a probation
officer or an attorney.

Fees:
• Fees for court ordered evaluations range from $100-$130.
• Expedited reports are available within 24 hours of the
evaluation for an additional $30 fee.
• Payment is expected at the time services are rendered and I
accept payment by cash, all major credit cards and debit cards
(no personal checks please).
• Please note that additional charges will apply to extra
paperwork required for some out of state evaluations.

limited to) the following issues: personal growth, stress
management, self-esteem, depression, anxiety, PTSD, anger
management, substance abuse, spirituality, relationship issues,
divorce, grief and childhood abuse issues (physical, emotional and
sexual).

What to
expect when you come in for your first session:
….My first priority to my clients is to create an environment
that is safe, comfortable, respectful, caring, positive and full
of hope. I will always keep the client’s best interest in mind.
If you are coming in for an individual counseling session, you
will fill out a few forms before we begin and then we will go
through an assessment. The assessment helps me get to know you
and it gives you a chance to share with me your reasons for
seeking counseling. The session will last an hour.
I offer e-mail support between sessions (within reason of
course).

Fees:
• Fees for individual counseling sessions are $100 per one hour session.
• Payment is expected at the time services are rendered and I
accept payment by cash, all major credit cards and debit cards
(no personal checks please).

If you require a DHR Approved Clinical
Evaluation due to being charged with 2 DUI’s or more within the same
5 year period in the state of Georgia, you will first have to
complete the DUI School and have them fax to me
the following information:

Your
NEEDS Assessment results (approximately 6-7 pages). Please
note that you can not bring this information with you, it has to
be faxed directly to me from the DUI School. You will
probably have to go to the DUI School and sign a release.

Paperwork from the DUI Schools can be
faxed to 404/250-6261. My DHR Clinical Evaluator number
is 38400. Please call me to make an appointment once your
paperwork has been faxed.

If for some reason you do not have all
the paperwork ready, we can still meet for an evaluation, however,
I will not be able to complete the evaluation and/or
recommendations until I have all the paperwork needed.
Please
bring with you the following information:
1.
Your letter of
completion (from the DUI School).
2.
You will also need to
bring a copy of your seven (7) year driving history/record. You can
get this from the DMV. You can go to
www.dds.ga.gov
We can then schedule an appointment to
meet for approximately 1-2 hours to complete the evaluation.

Note:
Please let me know if you have completed any substance abuse
counseling prior to your evaluation.

Most likely, if you
have been charged with a DUI, other drug or alcohol related charge
or anger/violence related charge, you will be required to undergo an
evaluation or assessment. If at all possible, it is in your best
interest to undergo this evaluation before your court date. I am
aware of at least two important reasons for this:

It shows the
court (or judge) that you are taking responsibility for your
actions (which makes you look good).

It can get
you through the system a little faster since you have already
started the process (this also makes you look good).
Some clients
hesitate to do this because they fear that they will automatically
be recommended treatment. This is not the case with the evaluations
that I provide. I do not believe that every person who has been
charged with a DUI or other drug or alcohol related charge has a
substance abuse problem, therefore, not everyone is recommended
treatment. However, sometimes this could be an indication that an
individual is either currently having a problem or they could be at
high risk for developing a problem in the future.
As a client, you
always have the right to get a second opinion if you disagree with
the evaluation results or recommendations. You also have the right
to complete any treatment recommendations at your facility of
choice.
